ICL Guide
ICL vs SMILE eye surgery
An overview of how ICL and SMILE differ, to help you prepare questions before a consultation. Suitability depends on your individual examination and a physician's assessment.
About this article
This article outlines factual differences between two treatment options to help you prepare questions before a consultation. It does not represent a recommendation for either option. Whether one is more suitable depends on your individual examination results, medical history, and a physician's judgment.
What SMILE is
SMILE (Small Incision Lenticule Extraction) is a type of laser refractive surgery that removes a small disc of corneal tissue through a tiny incision, without creating a corneal flap. Unlike LASIK, it does not require a flap. Unlike ICL, it reshapes the cornea rather than placing a lens inside the eye.
Key differences from ICL
ICL places a collamer lens inside the eye without cutting the cornea. SMILE reshapes the corneal tissue and does not implant anything. ICL can sometimes be removed or replaced; SMILE permanently changes the cornea. Whether one is better depends on your corneal thickness, prescription level, eye anatomy, and medical judgment.
When each may be discussed
Clinics sometimes present SMILE for people who want laser correction without a corneal flap, or who have concerns about flap-related complications. ICL is more often discussed when the cornea is thin, the prescription is high, or laser surgery is not suitable. Some patients are candidates for both; others may only qualify for one.
Availability and cost in Japan
SMILE is available at some eye clinics in Japan, though fewer locations offer it compared to LASIK. ICL is offered at a wider range of specialist eye clinics. Both are private treatments not covered by Japanese public health insurance. SMILE pricing is often between LASIK and ICL, but individual clinic pricing varies. Confirm whether your preferred clinic offers SMILE before comparing costs.
Questions to ask
Ask whether your clinic offers SMILE and whether it is recommended for your specific prescription and eye anatomy. Request an explanation that covers ICL, SMILE, and LASIK so you can compare them. If you need English or Chinese support, confirm it is available for the detailed comparison appointment and for follow-up visits.
